Release of Information

In order to help us serve you, your primary care team will need copies of your past medical history for conditions not treated at the James A. Haley Veterans’ Hospital.  A Release of Information Form is used for this purpose.  You can use this same form to obtain information to send to outside sources (e.g. insurance companies, employers, other physicians, etc.). Please contact the Release of Information Office with specific questions or for a copy of this form.
