Hospice / Palliative Care
| Hospice and Palliative Care (HPC) is all care in which the primary goal of treatment is comfort rather than cure in a person with advanced disease that is life-limiting and refractory to disease-modifying treatment; this includes bereavement care to the veteran’s family. Hospice care and palliative care collectively represent a continuum of comfort-oriented and supportive services provided in the home, community, or inpatient settings for persons in the advanced stages of incurable disease. Hospice and palliative care optimize the comfort and dignity of the patient through the effective management of pain and other symptoms, and by offering emotional and spiritual support when needed.
VISN 8 facilities are executing the commitment to ensure that the continuum of care, across all settings is accessible for any veteran who is in need of end of life care. A VISN workgroup, which includes representatives from all facilities, was established to standardize services and to develop guidelines for practice and implementation within our network. The intent of the workgroup is to make it easier for veterans at all sites to access hospice and palliative care services. Action plans have been created by the workgroup to meet the following goals.
